By signing this application I acknowledge that I understand that teacup puppies (adult weight under 4lbs) and Tiny Toy uppies (adult weight between 4lbs and 6lbs) need special care. I have read, understood and agree to the ‘Teacup & Tiny Toy Care’ as found on our website. 
 
By signing this application I acknowledge that I am 18 years old  or older and that I understand and agree that the $100 deposit to reserve the above mentioned puppy is non-refundable.   Puppies not picked within 4 weeks after their "adoptable date" become the property of 'Dallys Dogs' again.
